Headaches that cluster around menstruation are common, but they are not all the same. Hormonal fluctuation can lower the migraine threshold, while sleep loss, missed meals, dehydration, stress and head–neck tension may amplify the pattern. Magnesium may help some people, but it is not a substitute for diagnosis.
Is it menstrual migraine or another headache?
Menstrual migraine generally refers to migraine attacks that occur in a consistent window around menstruation. A throbbing or one-sided headache, nausea, light or sound sensitivity and worsening with activity suggest migraine, but tension-type headache, medication overuse and other causes can overlap.
Why can migraine worsen before or during a period?
A fall in estrogen near menstruation may reduce the migraine threshold in susceptible people. The hormonal change is only one part of the picture. Poor sleep, irregular meals, dehydration, caffeine change, stress and pain from menstruation can add to the total load.

Safety, supplementation and red flags

Use a three-cycle headache diary
Record the first day of bleeding, headache onset and end, severity, associated symptoms, medication, sleep, meals, caffeine and unusual stress for at least three cycles. A clear record helps distinguish a menstrual relationship from a pattern that only feels monthly.
